Prescription Diet Request Form

In order to serve you better and to help you navigate through your busy day, please use this on-line service to order prescription diet for your pet. This service is for current Laurelhurst Veterinary Hospital clients with pets on prescription diets as prescribed by our veterinarians.

The prescription diets are ordered weekly but please order the needed diet at least 1 week before you run out of your current supply. Food orders are placed on Mondays (unless it is a holiday); Hills, Royal Canin and Iams are usually received on Tuesdays and Purina on Wednesdays. Deliveries are also affected by holidays and may be delayed. Please call us if you have any questions about ordering prescription diets.
